Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I)prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief,certify that this project is consistent with the North Carolina Coastal Management Program.

I received your fax of a Notification/Waiver Form,With Mr.Ken Kirby name and 109
South Shore Dr. filled in. I cannot sign this Form its states there,is attached drawings and
dimensions showing the referenced property and proposed work to be done.And sent by
Certified Mail. Neither of the above was done. I need the proper drawing of the Bulk
Heads Location on the lot with detail of property line between me and Mr.Kirby and
how you propose to transport materials and equipment between my property line and the
5 to 6 feet to Mx.Kirby's house. Again I don't have a problem with Mr. Kirby building a
Bulk Head I plan to build one on my lot at some point.But as I have told you and Mr.
Kirby by phone and by fax my concern is my septic tank and lines being only 5' from our
property tines.Again any one traveling over or on my property will be charged with
trespassing.My address is
