Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become

This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I)prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available

Dear Sirs:
My wife and I would like to add a lift for our boat to the end of our
existing floating dock.With the addition of the lift we will have spaces
for two boats. Currently we only have space for one boat because the
land side of our floating dock is too shallow to dock a boat.We do not
even have cleats on the back of the floating dock. There are also some
old oyster shells behind the dock that makes it impossible to use that
side for docking. We have reached an agreement with Ed Flynn of'F&S
Marine to install the lift once we have a permit in hand. I am including
a survey that was done when we built our house two years ago.The pier
and floating dock were in place at the time the survey was done.The
survey indicates that with the addition of the lift we should easily meet
the setback requirements. Our home is located at 318 West Creekview Dr.
on lot 64 in the Deerfield Subdivision. On the survey this section of the creek
is referred to as Deep Bottom Branch.Thank you for your consideration of this
matter.
