Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I)prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief,certify that this project is consistent with the North Carolina Coastal Management Program.

Stoney Mathis issued the subject permit for the riprap fill along 142rof our property's shoreline that has had
aggressive erosion since the beginning of the year. The original contractor of record, Charles Fisher(General
Permit#35157D was issued by Joanne Steenhaus for the initial 75'of riprap fill) had an accident and could not
complete the work under Permit#37835 either by the July 28,2004 deadline or at any time in the future. I have
located another contractor, D.L. Construction, who will perform this work in several weeks.
Your office said you have taken over Stoney's workload in the short term and will have to re-visit the site as part of
the renewal process.This has been somewhat confusing having first worked with Joanne and then Stoney; Sue
McLaughlin said you've been involved behind the scenes pretty much since the beginning so there has been a
goodly amount of continuity.
The scope of work has not changed. Do I have to fill out more forms or just send in another$100 permit fee? Let
_ me know if I need to make a trip from Raleigh to be at the site when you go there. Thanks for helping me to
expedite the paperwork any way you can.We've been lucky so far hurricane-wise and want to finish protecting
