violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become

This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I)prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).

Dear Ms. Phillips:
I have responded to the notification I recieved by certified mail regarding Jack and Ruth's enlarged
dock.
While a new 16' foot dock will eliminate 2' of access to my boat dock which is parallel with the canal it
is livable. This would not be the case if the dock were not centered on their lot and entered the 15'set
back distance.
The drawing is inadequate to define just what construction is proposed and has no scale or
idendification other than 143 Starfish. If the dock is centered and 16 actually means 16 feet there should
be no problem.
Since the Nethertons are excellent neighblors whom June and I know very well, I have advised them
of our decision regarding the waiver.

Dear CAMA Representative:
I am submitting my wish not to waive the 15'setback requirement for
the proposed "New bulkhead, ramp, and float"as shown in diagram attached to
the Waiver Form I received by certified mail on 6-07-04.
My rationale for this refusal is the attached drawing is totally
inadequate for me to understand what is being proposed
I have a boat lift on my property that does not violate the 15'setback at
142 Starfish at my neighbors request(which I understand) and a dock that
leaves less than 30 feet between my facility and the proposed dock will make it
most difficult for me to enter my facility.
I would be happy to reconsider this decision (because the Nethertons are
excellent neighbors and I want to be helpful) if I am provided a layout with a
scale and some further identification data on what is really proposed.
