I request that a permit be issued to authorize the work described by the attached plans. I
represent the property owner on which the work is located and/or which is adjacent to the
water.
The adjacent property owners to this project have been furnished copies of this
application and related plans. The property name and address is located on drawing and
notifications.
You and I have talked about this and meet on sight.This is to replace a existing boatlift in
the same location. You indicated to me there would be no charge for this permit. If you
