Receipts will be forwarded to your office as soon as they are received. Also enclosed is a check
in the amount of$200.00 for the application fee.
This permit application supports our client's request to re-install a water line that was damaged
by a dredge so that water can be restored to this operating business. It also encompasses the
future installation of permanent water and sewer mains following government agency approvals
and notice of capacities.

The purpose of this form is to provide proper notice to you as an adjacent riparian property owner to the individual or
individuals listed below. 1'hc,CAMA General Permit application procedures require that applicants provide the Division of
Coastal Management confirmation that a written statement has been obtained signed by the adjacent riparian property owners

PROJECT NARRATIVE

PROJECT NARRATIVE
This project located at 1500 Point Harbor Road in Wilmington, North Carolina, will be
divided into two phases:
Phase I consists of the re-installation of a line which was recently damaged by a
dredge. This phase will involve the immediate installation of a temporary 2-inch
polypropylene water line which will be anchored to the river bottom using cast
