The purpose of this form is to provide proper notice to you as an adjacent riparian property owner to the individual or

"coastal Management confirmation that a written statement has been obtained signed by the adjacent riparian property owners

Enclosed are our Adjacent Riparian Property Owner Notification/Waiver Form
and a drawing of the dock we would like to build. There will be a 6 foot walkway
out to the dock. The dock will have a boat lift on one side and a covered area on
the other side.
If you have no objections to our proposed dock, please initial the appropriate line
on the enclosed form. I do not believe we need a waiver of the minimum distance
as we are putting the dock almost in front of our house but if you do not object,
please initial the appropriate line there as well. Please sign, print and date the
form. I am enclosing a self-addressed stamped envelope for your convenience.
