Pease issue a General Permit for Larry Lachat to install a boat lift in an existing slip at
lLumina Club dock. This facility is located at 627 Waynick Boulevard. The adjacent
p perry owners have been notified and the"green"cards are attached. I signed
n tification form and the basic sketch is also attached. Larry Lachat has agreed to have
act as his agent.
I 1 plan to pay for the permit once it has been issued. Please let me know when it is
