Hi Holley,
As we discussed on the phone earlier, we are helping the Cape Fear Public Utility Authority with
rehabilitation of the Burnt Mill Creek Sewer (Phase 1). The rehabilitation project, which is under

wetland disturbance by using mats for heavy equipment. The Army Corps did require us to submit a
Nationwide Permit (#3) to document impacts, which we did.
During rehabilitation of the sewer, three problems were discovered. The Authority now proposes to
repair these three deficiencies before placing the sewer back into service. We have discussed this
with the Corps, and have again submitted an application for a Nationwide Permit #3. 1 received an
email of the approval for that permit from the Corps this afternoon after you and I spoke.
Attached is a description of the three repairs that need to be made. Also attached are several
figures, drawings, and photos to help explain the proposed repairs.
Time is critical for completion of these repairs, because the contractor who is performing the
rehabilitation work is responsible for the bypass pumping. At this point, we are not allowing the
contractor to shut down the bypass system for the rehabilitation, but that work is nearing
completion, and we would like to make the repairs as soon as possible so that we can shut down
this temporary system. We consider these repairs to be emergency repairs.
Please review this information and let me know if we need a CAMA permit. Thanks for your
consideration. Please let me know if you have any questions or need additional information. We
would be happy to meet you on site this week to look at the facilities and describe the proposed
work —just let me know if you would like to do that.
