The project was previously permitted under a CAMA General Permit. The permit was issued on April 7, 2011 (permit no. 57307) which expired on April 7, 2012. Therefore, the intent of this submittal is to reapply for another CAMA General Permit for the project. No modifications have been made to the project since the issuance of the first permit. Enclosed please find a copy of the original general permit, one set of full-size plans for the sewer improvements for the above referenced project and a check for the CAMA General Permit fee of $400. The plans include a plan and profile view of the stream crossing on Sheets C-1 and C-3. Additionally, a copy of each adjoining property owner notification and the signed certified mail receipts are included as an attachment herein. August 2, 2012 International Paper Corporate Real Estate 6400 Poplar Avenue Memphis, TN 38197 Re: Burnt Mill Creek Phase 2 Sewer Improvements Project Division of Coastal Management Adjacent Riparian Property Owner Statement To Whom it May Concern, This letter is to inform you that you are an adjacent riparian property owner to a construction project that will be occurring within the Coastal Shoreline and Public Trust Areas of Environmental Concern (AEC) area. In March 2011 you received the same notification, but because construction has been delayed we are required to notify the adjacent property owners again in accordance with the North Carolina Division of Coastal Management requirements. This notification is required by the North Carolina Division of Coastal Management when obtaining a general permit. The permit is required when installing utility lines in the coastal wetland, estuarine water, public trust areas and estuarine and public trust shoreline AECs. The construction project being permitted is the second phase of a two phased sanitary sewer rehabilitation project. The sanitary sewer being rehabilitated is the Burnt Mill Creek Outfall, which was constructed in 1965 and has since experienced significant degradation. The rehabilitation project will prevent the possibility of a future failure of the Burnt Mill Creek Outfall. Phase I was completed in May 2009. Phase II consists of rehabilitating 2,500 linear feet of below -ground, 48-inch, 42-inch and 24-inch diameter sewer and 10 manholes and 1 junction box. The rehabilitation that will be occurring adjacent to your property will be installation of a new 48-inch gravity sanitary sewer line and associated manholes. The new parallel sanitary sewer will be installed within the existing permanent sanitary sewer easement. This phase of the project is scheduled to begin in October of 20.12. The overall site plan for the Phase II work is included in Attachment No. I. Also attached is a North Carolina Division of Coastal Management Adjacent Riparian Property Owner Statement form.
