Enclosed, please find signed Notification/Waiver Forms, and a copy of the drawing that was provided
both adjoining property owners, for the replacement of an existing damaged floating dock located at the
above referenced address. I am submitting this information on behalf of my wife, Jenny C. Finch, and her
siblings, David Carlton and Julie Carlton, owners of the property.

fee is waived for such repairs.
