Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that [)prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

Good morning Pam and Keith,
I have asked Ryan Davenport with the NC Division of Coastal Management to visit our lot and review the
culvert currently installed between the house and north river. I have requested authorization to remove
and replace the culvert. (It has filled in and has several failures.) Mr. Davenport advised me that a
general permit is available for this activity and to be able to receive this permit, I need to notify my
waterfront neighbors and request that you check off " I have no objection to this proposal at the top of
the attached form. If you would kindly sign the form and return to me via email and regular mail, I
would appreciate it.
In conjunction with the culvert removal, I intend to clean out the ditch back to its original grade, this
maintenance activity does not require a specific permit. If you have any questions, please let me
know. Mr. Davenport is carbon copied on this email if you have any questions for him as well. thanks,
