Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officerwhen the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythat this project is consistentwith the North Carolina Coastal Management Program.

Contractor Agreement
THIS AGREEMENT made the 27th day of September, 2019, by and between Crystal Coast Marine
Contracting, Inc., Hereafter called the Contractor and Jeff & Marianne Jones, hereinafter called the Owner
WITNESSETH that the Contractor and the Owner for the considerations named agree as follows:
Scope of Work
The Contractor shall furnish all materials and perform all of the work on the property at: 197 Sea Trace Lane,
Newport, NC
Work Performed:
Remove and dispose of existing dock and platform damaged from Hurricane Florence. Construct
new 17' wide by 17' Deep concrete observation platform along bulkhead. Construct new S wide
concrete dock in the same footprint as the original dock. Construct new 17' wide by 17' deep
concrete platform at the end of the new dock. Supply and install new Davit Master 10K boat lift
with wireless remote.
All debris and materials will be barged to job.
Exclusions: water, electrical, boat house repairs.
Deposit of $40,000.00 due upon contract signing, to lock in price commitment and estimated
start date of December 9-16, 2019.
Additional payment of $40,000.00 due on start date of project.
Balance due upon completion:
CONTRACT PRICE;
The Owner shall pay the comr ctor for material and labor to be performed under the sum of
