This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

The purpose of the project is to replace the existing crossline structure and repair the roadway which was damaged by
floodwaters from Hurricane Florence. The goal is to restore safe travel on SR 1649 as well as to meet current hydaulic
standards.

Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
Replacement of the existing 30" x 50' corrugate metal pipe with a 97' x 3'3" x 49.5' Aluminum Box Culvert. Dewatering of the
construction site will be accomplished by installing temporary sheet piles and a bypass pump will be used to dewater the site.
Any effluent from the construction area will be directedto a silt bag for treatment prior to discharge back into the stream. The
project will require the use of excavators, dump trucks, graders and rollers.

package lobe complete. Items (a) — (0 are always applicable to any major development application. Please consult the application
instruction booklet on how to property prepare the required items below.

Washington Acres Road (SR 1582). The existing corrugated metal pipe culvert measures 30" in
diameter by 50' in length. The proposed aluminum box culvert replacement will measure 9'2" in
width by 3'3" in height by 50' in length. NCDOT will be requesting a CAMA Emergency General
Permit for the proposed work and are required to notify you as the adjacent riparian property
owner. Please indicate your position or viewpoint on the proposed project and return the completed

Dear Mr. Lane:
The North Carolina Department of Transportation (NCDOT) is proposing to replace the existing
30" in diameter by 50' in length crossline pipe with a new 9'2" x 3'3" x 49'6" Aluminum Box
Culvert on SR 1649 Dogwood Lane approximately 0.42 miles SW of SR 1582 Washington Acres
Dr. The shoulders and pavement above the existing crossline were washed out during Hurricane
Florence and are currently only temporarily repaired. The proposed new culvert will be realigned
so that it is located entirely within the NCDOT R/W. NCDOT is requesting DCM approval of the
pipe replacement via CAMA Emergency General Permit 07H.2500. An adjacent riparian property
owner notification was sent via certified mail return receipt requested to Gary Sample at 351
Dogwood Lane. Frances Young was able to be contacted via telephone on October 8th and gave a
verbal statement of no objection.
Please find attached MP -I, MP-5, Site Plan View, Cross -Section, and copy of adjacent property
owner notification and certified mail receipt.
The department anticipates that a permit fee of $400.00 will be charged to the subject WBS
element number. Please let me know if I can provide any further information.
