I have attached the plan and profile of the crossing Spectrum plans to do in Bouge Watch
Subdivision. Also there is the MP_1P application, authorization agent form, property cards for the
4 lots involved and a detail of the silt fence.
I did not include a separate property owners list because they are all the same being unsold lots
at this time. The lots are listed on the MP_iP application along with the owner.
The letter will go out tomorrow for the owner and when the green card comes back I will send it
along. If there is anything else I need to do just let me know.
I usually pay for the permit when we meet on site but if that is not necessary just let me know
where to mail it or how you would like me to get it to you.

Along Fishermans point a total of 388 It Directional Bore of coax cable. Purpose is to provide service property owners. Once
the cable is is the ground no access is required unless there has been damage by others

package to be complete. Items (a) — (0 are always applicable to any major development application. Please consult the application
Instruction booklet on how to properly prepare the required items below.
a. A project narrative.
b. An accurate, dated work plat (Including plan view and cross -sectional drawings) drawn to scale. Please give the present status of the
proposed project. Is any portion already complete? If previously authorized work, clearly indicate on maps, plats, drawings to distinguish
between work completed and proposed.
c. A site or location map that is sufficiently detailed to guide agency personnel unfamiliar with the area to the site-
d. A copy of the deed (with state application only) or other instrument under which the applicant Calms title to the affected properties.
e. The appropriate application fee. Check or money order made payable to DENR.
I. A list of the names and complete addresses of the adjacent waterfront (riparian) landowners and signed return receipts as proof that such
