My name is Jeff Buchanan and I have a existing dock that was destroyed by Hurricane
Florence, there for I'm requesting a storm permit. My mailing address is 122 Dogwood Dr. Swansboro,
NC 28584 (Carteret County), the address on the Deed is 108 Dogwood Dr. I'm sending a attachment of

Lease to Purchase Agreement
Property Address: 108 Dogwood Drive
Swansboro, NC 28534
Sales Price: $125,000.00
Contract Date Begins: July 1, 2013
Contract End Date: open ended
This agreement has been set up between Jeff Buchanan (buyer) and Matt and
Danielle Mead (sellers). The terms as stated below are agreed upon by all parties
and cannot be modified without approval and signatures from all parties.
Jeff Buchanan agrees to pay $650.00 per month. Each time this payment is
received $143.50 will be credited towards the cost of purchasing the property.
Jeff Buchanan agrees to pay four installments of $2,500.00 each in August 2013,
November 2013, February 2014 and May 2014. These payments are considered
deposits towards the sales price of $125,000.00. All payments made will be
credited to the buyer and will reduce the amount owed to complete the purchase of
this property. In May 2014 another payment schedule can be arranged or buyer can
pay remaining balance at any time he wishes.
If at any time buyer decided not to go forward with the purchase of the property all
monies collected by the seller are non- refundable.
If the seller decided to forfeit this agreement all deposits would be refunded.
However, the $143.50 credit per month is not refundable under any circumstances.
