VMS # 1.2048111 Dear Mr. Daisey, The North Carolina Department of Transportation is requesting the issuance of a GP .2300 permit for the replacement of a culvert on SR 1146, Makleyville Road near Scranton in Hyde County. The culvert is located approximately 1.1 mi. east of SR 1145 on an unnamed tributary to Church Creek off Slades Creek and the Pungo River. The existing 36" X 34' CMP will be removed using NCDOT's guidelines for culvert removal. The proposed 36" X 40' CAP will be placed on the existing alignment as shown in the permit drawings with riprap for stabilization. NCDOT is also coordinating with the USACOE and NCDWR to acquire a nationwide permit for culvert replacements. Included with this letter are the location map, permit drawings, MP-1 and MP-5 forms and adjacent landowner notice cards/statements. This project will facilitate the conveyance of tidal water and drainage under SR 1146 while providing a safe passage for the traveling public. c. The culvert will be replaced using NCDOT best management practices and construction techniques for culvert replacements. The culvert will be replaced on the existing alignment. The culvert will be installed using an excavator, dumptrucks, and various other types of equipment. d. List all development activities you propose. The culvert will be entirely removed using NCDOT guidelines for pipe removal. The existing 36" X 34' CMP will be replaced by a 36" X 40' CAP, as shown in the provided permit drawings. Once the culvert is in place and back filled, a new roadway surface and grassed shoulder will be established. The banks will be stabilized following NCDOT erosion control bmp's. e.
