WBS number: 15403.1067800 Dear Mr. Lane: The North Carolina Department of Transportation (NCDOT) is proposing to place Class II riprap adjacent to the existing Onslow County Bridge # 46. The concrete apron of the bridge structure was damaged by Hurricane Florence. Broken pieces of the existing concrete apron which sloughed off into the creek after Hurricane Florence will be retrieved. Clean pieces will be added back to the bank for stabilization and covered with Class II riprap. Please find enclosed the completed DCM MP-1 and DCM MP-2 application forms detailing NCDOT's proposal under the Sections 07H. 1100 General Permit. In addition to the MP-1 and MP-2 applications, please find attached: 1. Copies of the letters sent to the adjacent riparian property owners 2. Original signed and returned green cards from the certified mail notice 3. Vicinity map and permit drawings detailing the project impacts. The bridge carries traffic on US-17 southbound over Northeast Creek. c. A turbidity curtain will be used to quarantine the construction site. Broken pieces of existing concrete apron which sloughed off into the creek after Huricane Florence will be retrieved. Clean pieces will be added back to the bank for stabilization and covered with Class II granite rip rap. The rip rap will be pushed into the sediment so that the rock is "keyed" in and will extend an average of approximately 6' beyond the Normal High Water Mark.The operation will be assisted by a long arm excavator, operated from the roadbed or bridge deck. Several dump trucks will be utilized to haul the riprap. Equipment will be stored at NCDOT's maintenance yard in Jacksonville, NC. d. Clean pieces of sufficient size will be placed bank on bank. - Placment of Class II granite riprap for bank stabilization. e.
