This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available

Subject: Application for 07H.1600 General Permit for installation of an 8" waterline beneath "The
Swash" on SR 1145 Village Point Rd.

Dear Mr. Lane:
The North Carolina Department of Transportation (NCDOT) is proposing to install a new 8"
waterline beneath "The Swash" which crosses under SR 1 l45 Village Point Rd. SW in Shallotte,
Brunswick County. The existing structure consists of 3 @ 48" RCPs. The waterline installation will
be performed by means of direction boring under the creek and surrounding wetlands
(approximately 10' beneath the bottom of the existing structure) for approximately 190 LF and will
require open trenching for the bore pits (approximately 35 LF on either side) which will be
constructed entirely on high ground. No permits are required from the N.C. Division of Water
Resources and a Nationwide Permit 12 has been issued by the U.S. Army Corps of Engineers and
is included in this application package.

8" by 255' utility pipe in place.

The proposed 8" waterline will be installed via 190 LF of directional boring beneath "The Swash" and surrounding wetlands. The

In addition to this completed application form, (MP-1) the following items below, if applicable, must be submitted in order for the application
package to be complete. Items (a) — (f) are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.

b. An accurate, dated work plat (including plan view and cross -sectional drawings) drawn to scale. Please give the present status of the
proposed project. Is any portion already complete? If previously authorized work, clearly indicate on maps, plats, drawings to distinguish
between work completed and proposed.
c. A site or location map that is sufficiently detailed to guide agency personnel unfamiliar with the area to the site.
d. A copy of the deed (with state application only) or other instrument under which the applicant claims title to the affected properties.

As required by the NC Division of Coastal Management Regulations, you are being notified as an
adjacent riparian property owner that the North Carolina Department of Transportation (NCDOT)
plans to relocate and 8" water line along SR 1145 (Village Point Road SW) via directional drilling
beneath "The Swash" (creek) located approximately 0.12 miles south of SR 1148 (Bill Holden Rd.
SW) in Brunswick County, NC. The attached plan sheets depict the proposed water line
installation. Please indicate your position or viewpoint on the proposed project and return the
completed form and any comments to: Mr. Stephen Lane, Division of Coastal Management, 400
Commerce Ave., Morehead City, NC 28557 or call (252) 808-2808.
Should you have objections to the project, your comments must be received within 10 days of
receipt of this notice. No response within 10 days will be considered the same as no objection if
you have been notified by certified mail.
