Any violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become null and void. This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. JUN 13 2013 DESCRIPTION OF PROPOSED DEVELOPMENT (individual proposal#M-MHDCt7'Y development must fill in description below or attach a site drawing) There is an existing vertical bulkhead fronting the water on the new 213 Smith Street lot. The proposal is to armor the front of this existing bulkhead with concrete slabs stacked parallel against the existing bulkhead angling from about 4 feet out to the existing timber beam holding the existing vertical bulkhead. The concrete slap heights will increase as they step toward the bulkhead with a projected 35 degree angle. This will help diffuse the wave action against the bulkhead armoring this bulkhead as it will reduce shear stress of the wave action against the bulkhead during storm events. This will help protect said property as well as the adjacent properties from bulkhead failure. WAIVER SECTION I understand that a pier, dock, mooring pilings, breakwater, boathouse, lift, or groin must be set back a minimum distance of 15' from my area of riparian access unless waived by me. (If you wish to waive the setback, you must initial the appropriate blank below.) I do wish to waive the 15' setback requirement. I do not wish to waive the 15' setback requirement.
