Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythat this project is consistent with the North Carolina Coastal Management Program.

Ellis to do this work. We authorize Eric Ellis to apply for and receive the permits required to

SUBJECT: Waterfront installation at 139 Beechwood Dr, Pine Knoll Shores, NC 28512
John & Lori:
We plan to install a boat hoist, dock and PWC lift on the waterfront. We have contracted with a
company that specializes in this type of work and plan to begin construction when all permits and
authorization requirements are met. We are required to inform all neighbors of our intention to add
to the waterfront and to provide a drawing of our plan.
Below is a drawing of our intended development on the waterfront. Your property is located to the
left side of the drawing. In this drawing the walk way will only be 3' wide rather than the 6'
indicated. We intend to have a boat hoist, a floating dock, and a swinging PWC lift as indicated. The
contractor will get all necessary permits and insure that we are in compliance with the offset
requirements. We intend to locate the swinging PWC lift on a piling on the bulkhead 10 feet from
your property line. This requires your concurrence. If you have any questions or concerns please
feel free to contact me on my cell phone line at (919) 606-5330.
We plan to begin construction as soon as all permits and authorizations are in place. We require
concurrence from you to begin construction.
