DESCRIPTION OF ACTIVITY AND CONDITIONS: .
ACTIVITY: To ictstall a septic system and erect frame house as per the submitted drawings.
CONDITIONS: Prior to initiating miy land-distLi ftig activities,.a barrier luxe of filter d46, staked hay
bales, or burlap must be installed bawem the land-distttnbingartivity and the adjaceint marsh or *ater
areas, until such tiwe as the area has been properly stabilized with a vegetated cover. A,tiy chAiW an die
proposal shall be re-evaluated bythe CAMA Officer. An Environmental Health septic system permit is
required. Iustallation must be done im accordance with the pennlitted workplat originally submitted on
August 16, 1999. This certification of exemption from requiring a CAMA Pennit is valid for ninety day;
from the date of isstiannce. Followung expiratiotl, a review of the.project and project situ may be nnocessary
to renew this cet tificatian.
