DESCRIPTION OF ACTIVITY AND CONDITIONS:
ACTIVITY: To install a septic system and to clear trees on property as per the submitted drawings.
CONDITIONS: Prior to initiating any land-dist. irbjng activities, a barrier line of filter cloth, staked hay
bales, or burlap must be installed.between the land -disturbing activity and the adjacent marsh or water
areas, until such time as the area has been..pwpe.rly stabilized with a y!�getated cover., Any.change in the
proposal shall be re-evaluated by, tlie..CAMA Officer. An Environmental Health septic system permit is.
required. Installation must be done in accordance with theperinitted workplat originally submitted on
August 10, 1999. This certification of exemption from requiring.a.CAMA Permit is valid.for ninety days..
from the date of issuance. Following expiration,.a review ofthe,project and project site may be necessary
to renew this certification.
