This permit, issued on 10 / 4 / 04 is subject to compliance with the application and site drawing (whore consistent
with the permit), all applicable regulations and special conditions and notes set forth below. Any violation of these terms may subject
permittee to a f)ne, imprisonment or civil action, or may cause the permit to be null and void.
1) All proposed dcvelopment and associated construction mast be done in accordance with the
application dated September 16, 2004.
2) All oonshuction must Conform to the NC Building Code regmreme:ots and all Local, State,
and Federal regulations.

require a re-evaluation and modification of this permit

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days of the issuing date.
From the date of an appeal, any work conducted under this per-
mit must cease until the appeal is resolved.

permit officer when the project is inspected for complianco,
Any maintenance work or project modifications not covered
under this permit requires further written permit approval.
All work must cease when this permit expires on December
31, 2007 w�

PERMIT
Pursuant to NCGS 113A-119(b), Pine Knoll Shores, a locality authorized to issue CAMA permits in Area
of Environmental Concern, hereby gives notice that on September 16, 2004, Russell F. Pagels applied for a
CAMA permit to construct a single family dwelling and build a walkway and bulkhead at 144 Arborvitae
Drive, Pine Knoll Shorts, NC. The application may be inspected at the address below. Public comments
received by September 29, 2004 will be considered. Later comments will be accepted and considered up to
the time of permit decision. Project modifications may occur based on further review and comments. Notice
of the permit decision in this matter will be provided upon written request.
