Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

Seed and mulch all disturbed areas. Maintain proper traffic control at
all times. Edge of bore pit shall be no closer than 5 feet from the edge of
pavement. Remain 3 feet below pavement and 2 feet below ditches.
Excavation within 500 feet of a signalized intersection will require notification
by the party of the second part to the Division Traffic Engineer at telephone
number [phone]. All traffic signal or detection cables must be located
prior to excavation. No open cut of pavement will be allowed. See page
two for further comments.

The party of the second part agrees to provide traffic control devices, lane closures,
road closures, positive protection and/or any other warning or positive protection
devices necessary for the safety of motorists and workers during construction and any
subsequent maintenance. This shall be preformed in conformance with the latest
NCDOT Roadway Standard Drawings and Standard Specifications for the Roads and
Structures and Amendments or Supplements thereto. When there is no guidance
provided in the Roadway Standard Drawings or Specifications, comply with the
Manuel on Uniform Traffic Control Devices for Streets and Highways and Amendment
or Supplement thereto. Information as to the above rules and regulations may be
obtained from the Division Engineer of the party of the first.
The North Carolina Department of Transportation is in the process of developing a
Work Zone Traffic Control Qualification Program that will begin its implementation in
2009. This program will require qualified and trained Work Zone Flaggers in every
flagging operation (July2009), qualified and trained Work Zone Traffic Control
Installers on every traffic control installation (January 2010), and qualified and trained
Work Zone Traffic Control Supervisors on Significant Projects (July 2010). It is
intended for the program to include anyone working within the NCDOT right of way
including work associated with NCDOT construction and encroachment agreements as
well as all NCDOT operations.
Training for this certification will be provided by NCDOT approved training sources
and/or private entities that have been pre -approved to train themselves. Additional
information will be provided as this program progresses. If you have questions, visit

The NCDENR (CAMA) requires that adjacent riparian landowners are to be notified of work taking place
on adjoining property as a part of the CAMA permitting process. Please consider this letter as notice that
CenturyLink has applied for a CAMA permit to construct buried telephone cable along Hwy 101 that
adjoins your property. Adjacent property owners can provide any comments on the proposed
work, in writing, for consideration by permitting officials to the Division of Coastal Management, Attn:
Brad Connell, 400 Commerce Ave., Morehead City NC 28557 within ten days of receipt of this notice. No
response will be interpreted as no objection. DCM staff will review all comments and determine, based on
their relevance to the potential impacts of the proposed project, if the proposed project can be approved by
a General Permit.
