(Waterbody) (Town and/or County) They have described to me, as shown below, the development they are proposing at that location, and I have no objections to this proposal. I understand that a pier/mooring pilings/boatliftlboathouse must beset back a minimum distance of fifteen feet (15') from my area of riparian access unless waived by me. XI do wish to waive the setback requirement. I do not wish to waive the setback requirement. DESCRIPTION AND/Olt DRAWING OF PROPOSED DEVELOPMENT: (To be filled in by individual proposing development) The development consists of a deck and stairway down the embankment to a dock that begins at the base of the slope. The 4 foot wide dock extends straight out from shore 184 feet, at the end of which is a 16 foot by 12 foot deck area. This dock and deck area extends into the water no more than the nearest dock/decks of my neighbors on both east and west sides. On the west side of the dock, inshore from the deck area, is a single pole lift suitable for a personal watercraft or johnboat. The main boatlift is on the east side of the dock, adjacent to the deck area. The boatlift area is covered by a simple pitched roof that is 12 feet by 24 feet. The south and east sides of the boatlift area is bordered by a 3 foot wide walkway.
