Attached is General Permit #44293C- this permit authorizes the reconstruction of the 12' x 16'
"T" platform in it original footprint.
In order to validate this permit, please sign the permit as indicated. Retain the white copy for
your files and return the signed yellow and pink copies to us in the enclosed, self-addressed
envelope. If the signed permit copies are not returned to this office before the initiation of
development, you will be working without authorization and will be subject to a Notice of
Violation and subsequent civil penalties.

This is in follow-up to our discussion of this past Friday. In the Ophelia storm surge, we
lost the t-head of our pier at 2510 Emerald Drive. I am attaching photos and respectfully
request issuance of a permit to allow us to rebuild what we had. The work will include
new pilings (except for 3 — 4 that weren't taken out) and then building a frame and deck.
We don't know for certain because it was there when we purchased, we hadn't measured
and don't have a drawing, but we think the size was in the 12' x 16' to 16' x 20' range
judging by remaining pilings and discussion among those of us that used it. If you have
record on it, would appreciate an indication.
