This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

Per our telephone conversation concerning the above referenced project, we are requesting
a General CAMA Permit be issued for this project.
As discussed, we have designed a 4" HDPE Directional Drill Bore to be installed under
Wheeler Creek. As shown on the plan, on each side of the bore, the Contractor will then excavate
and install a 45 degree bend to route the main up to the road shoulder.
Please find enclosed an 8'/2 "X 11" Exhibit Map showing the Plan and Profile views of our
design and a check in the amount of $100.00 for the Permit Application Fee. Also enclosed are
copies of the Letters of Notification that we sent to the adjacent property Owners via Certified Mail.

Onslow County has hired our Company to design a 4" Waterline Extension to connect
together existing dead end waterlines that are on each side of the bridge crossing Wheeler Creek
on Wheeler Creek Road, NCSR 1557.
In order to cross the Creek, we are proposing to bore under Wheeler Creek using the
Directional Drill Bore Method. This method keeps the disturbance to the creek area at a minimum.
We propose to bore under the creek, then on each side of the creek, install bends and route the
waterline back up to the road shoulder to tie to the existing water mains, using the Conventional
Trench Excavation Method.
The County has to apply for and receive a CAMA permit prior to construction. The enclosed
form is required by CAMA to notify the adjacent property Owners that a CAMA permit is being
requested. If you have any comments or objections, you may complete the form and return it to
CAMA. Also, please find enclosed a copy of the plan showing the design of the Waterline
Extension.
For your information, we are also applying for a DOT Right -Of -Way Encroachment Permit
and for a NCDENR Waterline Extensions Permit.

EROSION AND SEDIMENTATION CONTROL NOTES:
I. Notify owner, Engineer and Onslow County Water Department
prior to beginning construction
2. Contractor shall install rock check dams and silt fence
prior to construction, in the locations shown on the plans
or as directed by County of 0nslow Inspector.
3. Rock check dams shall be placed in the roadside ditch on
both sides of storm drains or in outlet ditches to prevent
sediment from leaving the site.. Contractor will periodically
remove collected sediment and maintain rock check dam in
good working condition until all disturbed areas are stabilized.
4. All trenches shall be backfiiled, regraded, grassed, mulched
and restabilized as soon as practical to minimize erosion. All
areas shall be restabilized within 7 days of main installation.
Any areas that erode shall be regraded and regrassed. Any
sediment accumulating in the roadside ditches shall be
removed and ditches shall be regraded and regrassed.
5. Contractor shall keep all excavated dirt off of the existing
asphalt roads so as not to damage the road during cleanup.
Any damage to existing roads shall be repaired at contractor's
expense.
6. All disturbed shoulders shall be regraded and regrassed to
their original condition to the satisfaction of the Engineer
and / or Department of Transportation officials, if within D.O.T.
right-oF-way Sediment runoff into the adjacent roadside ditch
Shall be removed and the ditch shall be restored to its original
condition.
7. Contractor is responsible for maintaining all erosion control
measures and shall amend measures as regpired to prevent
accelerated erosion from taking place on this site.
8. Once areas have been stabilized, contractor is to remove
temporary erosion control measures, regrade, reseed and
remulch these areas as needed.
9. Seeding Specifications:
Apply lime and fertilizer and work into seed bed. Seed mixture
shall be distributed uniformly and covered with a clean straw
mulch. All mulch areas shall be crimped or tacked to stabilize.
Minimum material and application rotes are as follows:

"Rebel" Fescue 50 Ibs./ac.

12. Any additional erosion and sedimentation measures necessary to
stabilize disturbed areas and return areas to its original or
better condition and to keep sediment from leaving the site

WATER NOTES:
I. Potable Later supply to be by County of OnSIOw.
2. All construction within Department of Transportation
right-of-way shall be in accordance with N.C.D.O.T.
regpirernents. See D.O.T. encroachment agreement.
Contractor shall submit performance and indemnity
bond to Department of Transportation prior to
beginnira construction. D.O.T. officials shall be notified
48 hours prior to commencing work within D.D.T. rights -
of -way.
3. Locations and depths of existing utilities shown are
approximate only. Contractor is responsible for verifying
locations and depths of any utilities present. Contractor
is responsible for any damages he inflicts to existing utilities.
4. As of January 1, 1986, anyone digging with mechanized
egpipmcrit in highway right-of-way, private utility easements,
or publ c spaces will be respired by North Carolina law to
give na:ice of the proposed excavation to all existing area
utilities at least 2 working days before starting to dig. The
utility owner is to locate its facilities in the area of the
proposed excavation. Utility members of ULOCO can be
contacted at 1-800-632-4g49.
S. All water mains to be P.V.C. ASTM D-2241, ASTM D-1784,
ASTM D-1869, 200 P.S.I. pressure rating or ductile iron were
specified.
(o. Fire hydrants to be Mueller Centurion model A421 or
approved eapol.
7. Water r-)ains shall be buried with a minimum cover of 36-inches
unless -otherwise noted.
8. Water cnd sewer mains shall have proper separations. At water
and sewer crossings, the water main shall have 18 inches
separation above the sewer main or they shall be constructed
of ferrous material for a minimum of 10 feet each side of the crossing.
See specifications for further details.
9. Contractor to lay water mains at extra depth to avoid conflicts
with storm drainage or other facilities. See details.
10. Contractor shall notify the consulting Engineer and the Onslow County
Water Department at least 48 hours prior to commencing any work,
inspect,ons, sampling, and pressure tests.
11. Chlorination and pressure tests respired in the presence of the
certifying Engineer and the Onslow County Water Department.
12. Contractor shall furnish negative bacterial analysis results and
24 hou- chlorine residual results to the certifying Engineer.
13. Contractor shall maintain comprehensive (depth and horizontal
locations) field "As Builts" for all installations and submit them to
the Cc-tifying Engineer and the Onslow County Water Department.
14. AnythinGj disturbed shall be replaced to its original or better condition
to satisfaction of owner, Engineer, Department of Transportation
and County of Onslow.
15. Contractor has the option of pushing / boring uncosed main under
driveujoys or removing and replacing existing driveways and pipes to
original condition.
16. Contractor shall keep all work within D.O.T. right-of-way and not
disturb any property outside of right-of-way without property
owner'., permission.
Contractor shall coordinate with local utility companies to shore or
suppor+. existing power / phone poles and all appurtenances. This work
is to to included in the bid price.
15. Contractor shall install traffic control devices (lighted barricades,
cones, signs, and flagmen as respired) along all roadways affected by
Constriction per N.C.D.O.T. and County of Onslow Standards.
