This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .

SUBJECT: Replacement of Bridge 81 (TIP No. B-2924) on SR 1321, Brunswick County
Dear Mr. Arrington:
Please find enclosed a CAMA General Permit Application from the North Carolina Department of
Transportation for the replacement of Bridge 81 (TIP No. B-2924) in Brunswick County. A CAMA
General permit had been acquired for this project (GP #C-30569), however due to delays in getting a
contract completed, construction has not begun on this project and the current GP expires on 3/21/03.
Construction is now scheduled to begin April 2003. The scope of the project which includes the removal of
the existing three span 75.6 ft x 24.1-11 steel I beam supported reinforced concrete deck bridge with a three
span 120 ft x 27 ft prestressed concrete core slab deck bridge has not changed. However, the contractor has
requested authorization to construct a temporary work platform on either side of the existing end bents to
access the drill shaft sites on this project. Each proposed work platforms will extend approximately 10 ft
from the existing end bents and will be supported by two H-piles. The project will impact 50 linear feet of
streams associated with Wet Ash Swamp, which is classified as C Sw by the Division of Water Quality.
Stream impacts will be largely associated with the removal/installation of piles and installation of drill
piers.
A letter requesting modification to the existing ACOE Nationwide 14 permit (Action ID #200200568) to
include a NWP 33 for the construction of the work platforms has been sent to Dave Timpy with the Army
Corp of Engineers. Per the Division of Water Quality General Certification #3366 for NWP 33, no written
concurrence from the DWQ is required.
