Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythat this project is consistent with the North Carolina Coastal Management Program.

We realized that this slope will be subject to erosion and will be difficult to stabilize
without placing rip -rap on it. The additional rip -rap is shown on the attached plans. It will
not have any additional fill associated with it. The rip -rap will be placed on the permitted
fill slope at the NWL and then extend up the slope 2-3 ft.

Dear Mr. Hoadley,
The North Carolina Department of Transportation is requesting the issuance of a GP. 2300 permit for
the replacement of Bridge # 42 in Hertford County. The bridge is located on SR 1441 (Swains Mill Rd.)
approximately 3.5 miles east of Harrellsville. The bridge spans Deep Swamp Branch (Taylor Mill Pond),
which is a tributary that flows to the Chowan River approximately 0.15 miles downstream. The Division of
Water Quality classifies Deep Swamp Branch Creek as Class "C", "NSW" waters. Bridge # 42 is currently
a wooden structure that is 70 ft. long by 25 ft. wide. This structure will be removed using NCDOT's
guidelines for bridge removal. The proposed bridge will be replaced on an alignment that is located slightly
south of the existing alignment. The new bridge structure will be 110 ft. in length by 30 ft. wide and built
with precast cored slab concrete deck, support piles and bents. The bridge roadway approaches will have to
be reconstructed. This will result in some widening of the approaches. Impacts associated with this project
will result from 596.36 sq. ft of fill in 404 wetlands and 1651.99 sq. ft. of fill in surface waters. This fill
material is associated with the reconstruction of the approach shoulders and bank stabilization around the
end bents. The wetland impacts will be off set by removal of 450 sq. ft. of old bridge approach fill, which
will be graded to adjacent wetland elevation. An off -site detour will be utilized to detour traffic around the
project area.
