Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

Attached is General Permit #57908C to replace an aerial electrical utility line on SR 1763, Havelock, North
Carolina and #57907C to replace existing 28' X 105' bridge with a 30' 10" X 124' long bridge and associated
infrastructure and re -locate a water utility line on NC 99, approximately 2 miles north of SR 1625, in Pantego,
North Carolina.
In order to validate this permit, please sign the permit as indicated. Retain the white copy for your files and
return the signed yellow and pink copies to us in the enclosed, self-addressed envelope. If the signed permit
copies are not returned to this office before the initiation of development, you will be working without
authorization and will be subject to a Notice of Violation and subsequent civil penalties.

Line has to be Relocated in Order to Perform This Work.

For documentation: Since the urgency of this repair is due to safety reasons, WRC does not
object to NCDOT conducting this work during the requested Moratorium period. If it is
determined that access to the Boating Assess Area will be impacted let me know so I can
coordinate appropriately.

Subject: Craven 176 Crutch Bent
Good Morning Mr. Travis .... Hope all is well ... We have a scour problem on Craven 176 and we
will need to drive a crutch bent under the first span. This is an emergency situation; we
have to do this to keep the existing bridge open to the public. This operation will take
about 3 days. Utilities are being moved this week and we hope to conduct the crutch bent
installation next week. There will be no impacts or disruptions to the existing boat ramp. We
are aware of the February 15 to September 30 Moratorium. I've attached maps of the vicinity
