Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

Subject: Bridge # 20 in Bertie County Permit Drawing Revision
Bill, Garcy, Stephen
The Bridge department decided to modify the plan just slightly. At site 1 on this drawing
they elected to use rip -rap instead of a vertical sheet pile shoulder protection. This
resulted in a change of Ill sq. ft. of additional fill (rip -rap) in what was originally shown
in the hand cleared area at site 1. This change is shown on the attached plans. I also
included the old plan for your comparison.
If you guys need anything else on this let me know. We are having a precon on this bridge on
Wednesday.

The North Carolina Department of Transportation is requesting the issuance of a
General Permit .2300 for the replacement of Low Impact Bridge # 20 in Bertie County.
The bridge is located on SR 1235 approximately 1.65 miles west of the intersection of US
13. The bridge spans Stony Creek, which is a tributary to Ahoskie Creek. Ahoskie Creek
is classified by DWQ as class "C, NSW" waters. Bridge # 20 is currently a wooden
structure with a timber deck that is 76 ft. long by 26 ft. wide. This structure will be
removed using NCDOT's guidelines for bridge removal. The proposed bridge will be
replaced on its existing alignment. The new bridge structure will be 92' in length by 33'
wide and built with precast cored slab concrete deck, concrete piles and concrete end
bents. Jurisdictional wetland and surface water impacts have been minimized, resulting in
0.007 acres of permanent wetland impact and 0.016 acres of temporary wetland impact,
due to hand clearing for erosion control installation and maintenance. An off -site detour
will be utilized during construction. This bridge is following the Low Impact Bridge
Process established by FHWA, NCDOT and Regulatory Agencies.

This project will provide reliable and safe transportation along SR 1235
by replacing an existing bridge crossing over the Stony Creek. Once the bridge is replaced the roadway facility will continue
to provide connectivity and safe access for the traveling public living and traveling through this area of Bertie County.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
The bridge will be replaced using top down construction techniques utilizing precast concrete structures (piles,caps,deck).
The new bridge will be replaced on its existing alignment, with additional length and width to allow for current design
standards and safety improvements. The bridge will be constructed using a crane, excavator, dumptrucks, pavers and
various other types of equipment.
d. List all development activities you propose.
The existing 76' long x 26' wide bridge will be entirely removed using NCDOT guidelines for bridge removal. The new bridge
will increase the horizontal floodway by extention to a 92' long x 33' wide bridge. The new bridge will consist of 3 concrete
spans supported by concrete end bents and girders. This bridge will be replaced on the existing alignment, causing a
temporary road closure. Traffic will be detoured along available routes. Wetland Impacts have been avoided and minimized
to the maximum extent practicable. The resulting impacts are 409 sq. feet of permanent wetland impacts and 730 sq. feet of
temporary wetland impacts associated with hand clearing for erosion control device maintenance area. Approximately 526
sq. feet of bridge approach fill will be restored to flood lain elevations as shown in the bride profile drawing.
