This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .

Dear Mr. Lane,
The NCDOT request the issuance of a General Permit .2300 and .1600 for the
replacement of Bridge # 34 and boring of a water line in Hyde County. The bridge is located on
SR 1 148 (Sladesville Rd) approximately 0.18 miles south of the intersection of SR 1152. The
bridge spans Smith Creek, which is a tributary to the Pungo River. Smith Creek is classified by
DWQ as class "SC, NSW" waters. Bridge # 34 is currently a wooden structure with wood deck
that is 48 ft. long by 25 ft. wide. This structure will be removed using NCDOT's guidelines for
bridge removal. The proposed bridge will be replaced on its existing alignment. The new bridge
structure will be 57' in length by 30' wide and built with precast cored slab concrete deck,
concrete piles and concrete end bents. An off -site detour will be utilized during construction.
In conjunction with this bridge replacement a 8 in. water line will be relocated by boring
on the west side of the bridge as indicated on the permit drawings. The bore will total 300 ft. in
length and be set 3 ft. below creek bed. There will be no impacts associated with this boring.
The bridge replacement will result in 712 sf of permanent wetland impact associated
with the fringe coastal wetlands along the road shoulder. There is 72 sf of surface water impact
associated with fill slope extending into the existing roadside drainage features. Approximately
104 sq. ft of area will be graded to a lower wetland elevation around the bridge end bents. This
project is located in the Tar -Pamlico Buffer Basin, however all buffer impacts have been
avoided.
This bridge is following the Low Impact Bridge Process established by FHWA, NCDOT
and Regulatory Agencies. Included with this letter are the location map, permit drawings, MP-1

Provide safe transportation along SR 1148 by replacing an existing bridge crossing over Smith Creek. Once the bridge is
replaced the roadway facility will continue to provide safe access for residential homes and agricultural operations located
within the area.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
The bridge will he replaced , ising torn, down Construction techniques utilizing precast concrete structures (piles,caps,deck).
The new bridge will be replaced on its existing alignment, with additional width to allow for current design standards and
safety improvements. The bridge will be constructed using a crane, excavator, dumptrucks, pavers and various other types
of equipment.
d. List all development activities you propose.
The existing 48' long x 25' wide wooden bridge will be entirely removed using NCDOT guidelines for bridge removal. The
new bridge will increase the horizontal floodway by extention to a 57' long x 30' wide bridge. The new bridge will consist of 1
concrete span supported by concrete end bents and girders. This bridge will be replaced on the existing alignment, causing a
temporary road closure. Traffic will be detoured along available routes. Wetland Impacts have been avoided and minimized
to the maximum extent practicable. The resulting impacts are 712 sq. feet of wetland impact and 72 sq. feet of surface water
impact. Removal of the bridge approach fill will result in 104 sq. ft of wetland restoration.

Project Description — The NCDOT is proposing to replace an existing 48 ft. long X 25 ft. wide wood bridge with a
57 ft. long X 30 ft. wide concrete bridge. The new structure impacts have been minimized to the maximum extent
practicable. The roadway shoulder widening to accommodate the new bridge approach will result in 712 sq. ft.
(0.016 acres) of coastal wetland impact and 72 sq. ft. (0.002 acres) of surface water fill. This project has avoided
Tar -Pamlico buffer impacts. Do to the lengthening of the bridge structure, approximately 104 sq. ft. of wetland,
will be restored by removal of bridge approach fill to surrounding wetland elevation. NCDOT will follow all
guidelines and BMPs for the removal of the existing bridge and Erosion / Sedimentation Control.

Attached is General Permit 457914C to construct a 30' X 57' long bridge to replace the existing 25' X 48' long
bridge, and to replace a water utility line on SR 1148, in Scranton, North Carolina

copies are not returned to this office before the initiation of development, you will be working without
