Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

The pavement at the end of Harkers Island Road (SR 1335) has been undermined by wave and tide action, causing the
pavement to break off. The undermining action and broken pavement are isolated to the south eastern portion of the end of
the road, which is also the location of the primary outlet point for runoff from the roadside ditch running along the south side
of SR 1335.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.

Remove broken asphalt along the affected portion of the roadway, rebuild the underlying slope and armor with filter fabric
and class "B" granite rip rap. All work will be located within the footprint of the removed/damaged pavement and within the
limits of the rights of way. Also propose lining the lower portion (approx. 100 linear feet) of the existing roadside with fabric
and class "B" granite.

package to be complete. Items (a) — (0 are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.

b. An accurate, dated work plat (including plan view and cross -sectional drawings) drawn to scale. Please give the present status of the
proposed project. Is any portion already complete? If previously authorized work, clearly indicate on maps, plats, drawings to distinguish

c. A site or location map that is sufficiently detailed to guide agency personnel unfamiliar with the area to the site.
d. A copy of the deed (with state application only) or other instrument under which the applicant claims title to the affected properties.

This correspondence is to notify you as an adjacent riparian landowner that the North Carolina Department of Transportation
has determined that the pavement at the end of Harkers Island Road (SR 1335) has been undermined by wave and tide action,
causing the pavement to break off. North Carolina Department of Transportation plans to remove broken asphalt along the
affected portion of the roadway, rebuild the underlying slope and armor with filter fabric and class "B" granite rip rap.
