Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

Attached is a request for a permit to add an additional lift to an existing dock
located at 159 Banks Street in Morehead City. This dock is shared between myself, and
my next -door neighbors, Al and Gail Smy. The dock straddles both our property lines and
has been in place for years. There are two lifts in place, (one each for both neighbors).
These lifts have been installed in the last two years.
There are also several existing slips, one of which is currently serviced by an old
out of date lift. My intent would be to remove this older lift and install a newer
serviceable lift adjacent to my existing lift, as I now have 2 small boats. Enclosed are
permission forms from both my adjacent neighbors, photographs of the existing slip and a
proposed layout for the new slip.
Please let me know if there is anything else that I need to provide. Thank you for
