This permit is subject to compliance with this application, site
drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro-
ject is consistent with the local land use plan and all local
ordinances, and 2) a written statement has been obtained from
adjacent riparian landowners certifying that they have no
objections to the proposed work.

Dear Mr. Vick,
This letter is in response to the application for state permits dated 17 May 1996 that was
submitted to the Division of Coastal Management for replacement of an existing bridge with a
new bridge across a tributary to Broad Creek on SR 1331 in Beaufort County, North Carolina.
A review of the application package raised several questions which resulted in a recent
phone conversation with Mr. Gordon Cashin, N.C. DDT's contact person for this particular
project. Mr. Cashin agreed with my conclusion that clarification needed to be made in regards
to the submitted plats.
The following is a list of information that I need prior to accepting the application
package:
(1) Plan view plat - the normal water level needs to be shown on both sides of the
creek.
(2) Plan view plat - the lines which indicate the width and ends of the existing bridge
should be labeled as such.
(3) Plan view plat - the lines which indicate the width and ends of the proposed
bridge should be labeled as such.
(4) Cross section plat - the normal water level should be shown along with the
bottom most part of the existing and proposed bridge so that the navigation
clearance can be determined.
(5) Cross section plat - the existing piers are labeled, however, the proposed piers
should be labeled also.
(6) The certified mail return receipt (green card) should be submitted to show
notification of the one adjacent property owner (Boy Scouts of America) has
occurred.

As you are aware, for the past year the Division of Coastal Management has been
developing a general permit for the replacement of bridges and culverts which meet certain
conditions. This new general permit became effective on 1 June 1996. A review of the general
permit and a review of this current proposal indicates that this bridge replacement will qualify
for the issuance of a general permit. To get the general permit issued, I will need for you to
make the changes that I have requested above and to have an individual from DOT meet me at
the project site and supply to me the complete package. If the changes are satisfactory then I
will be able to issue the permit at the site an work can commence.
