Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

We hereby propose to furnish materials and labor necessary for the satisfactory completion of. BULKHEAD
Installation of approximately 86 ft. of Vinyl Bulkhead Wall with (2) 8' wingbacks. Vinyl wall will be approximately 4 ft. above
ground and 4 ft. in ground. Removal of existing retaining wall, haul debris to landfill. All necessary backfill and centipede sod
for all disturbed areas are included in this contract. All State and County permits are included in this contract.
All material is guaranteed to be -as specified, and the above work will be -performed in a workmanlike manner in accordance with the
swings and specifications submitted. All work will conform to all state, county, and local codes, permits, and guidelines. This
ontract can be performed for the amount of: $ 19,000.00
Payment schedule is to be made as follows:
$ 6,333.34 Deposit upon acceptance of contract. �2 (-Iq
$ 6,333.33 Payment due upon delivery of material.
$ 6,333.33 Final payment due upon the day of completion.

Any alteration or deviation from above specifications involving extra costs will be executed Repre nted by:
only upon written order, and will become an extra charge over and above the estimate. All
agreements are contingent upon strikes, accidents, or delays beyond our control. No extra
services will be performed without written consent. Note: This roposal may be withdrawn by us if not
accepted within 30 days.
ACCEPTANCE OF PROPOSAL
The above prices, specifications, and conditions are satisfactory and are hereby accepted. You are authorized to do the work as specified. Payments will be made
as outlined above. Before the job begins, the Owner shall furnish the Contractor with the location and character of any underground utilities not located by the
locating service. If Owner shall fail to do so, Owner agrees to release, indemnity, save harmless and defend the Contractor from and against all liability, loss,
damage, and expense caused or occasioned thereby. If Owner fails to complete the payment schedule as outlined and an attorney is hired for collection, Owner
agrees to pay all costs associated to attorney fees, court fees, and all other legal fees that may be incurred with the collection of this debt.
