Attached is General Permit #43483C, to replace pier and dock damaged in hurricane Ophaelia,
the structure must be re -constructed to previous dimensions and configurations, as per photos
submitted 10-18-2005.
hi order to validate this permit, please sign all three (3) copies as indicated. Retain the white
copy for your files and return the signed yellow and pink copies to us in the enclosed, self-
addressed envelope. If the signed permit copies are not returned to this office before the initiation
of development, you will be working without authorization and will be subject to a Notice of
Violation and subsequent civil penalties.

We spoke on the phone a couple weeks ago. I am sending you a picture of the pier and two
pictures showing the damage. I am requesting a permit to rebuild the pier as it was before the
storm and to add rip rap where erosion has occurred close to the foundation of the house and deck
stairs and pilings. Please notify me as soon as possible, when the permit is available.
