drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro-
ject is consistent with the local land use plan and all local
ordinances, and 2) a written statement has been obtained from
adjacent riparian landowners certifying that they have no
objections to the proposed work.

Per our phone discussion last Friday, enclosed is a check for $50 to
cover the cost of permits for Lewis Branch as follows:
Extend permit # 021495-C for the construction of a bulkhead for an
additional three months. Also amend that permit to cover the construction of
a new bulkhead behind the current existing bulkhead as shown on the permit
next to the Canal. This added section will be 150' long including the turn
back at the boat launch section. New bulkhead will be constructed behind
the current wall and the current wall removed after completion of the new so
erosion is prevented.
In reference to permit 4019909, please permit the maintenance excavation of
the canal / Lewis Branch entrance. The area needing maintenance is
approximately 100' long and 25' wide. Excavation would be consistent with
previous work to a depth of 2'. Only sand is being removed, expect no run
back from spoil and spoil area would be as shown on that permit. Sand
would then be removed after drying.
Attached is a signed agreement from Robert McDonald. In advance,
thanks for your assistance. If you need to contact me, I can be reached at

mail to me the proper permitting.

The individual applying for this permit to do maintenance excavation to the
entry to Lewis Branch has shown me the original permit 4019909 and
discussed the current planned excavation.
