This permit, issued on �/ DI-7 , is subject to compliance with the application (where consistent with the permit), all applicable regulations, special conditions and notes set forth below. Anv violation of these terms may subject - v-permittee to a fine, imprisonment or civil action; or may cause the permit to be null and void. (1) The proposed pier addition will be constructed in accordance with the attached project plans. (2) The permittee shall prominently post and enforce a "no sewage discharge" policy :for boats at the entrance and exit from the main -pier. (3) This permit authorizes only the docks, piers and other structures and uses located in or over the water that are expressly and specifically set forth in the permit application. No other structures, whether floating or stationary, may become a permanent- part of this pier system without permit modification. No non -water dependent uses of structures may be conducted on, in, or over public trust waters without permit modification. (4) The offshore platform of the pier must be adequately marker with navigational reflectors. (6) The activit;- will be conducted in such -a manner as to prevent \significant increases in turbidity outside the area of construction or construction -related dischar e. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty (20) days of the issuing date. An appeal requires resolution prior to work initiation or continuance, as the case may be. This permit must be accessible on -site to Dep::rtment personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Departmental approval. (8) The permittee -will maintain the authorized work in good condition in .and conformance with theterms and conditions f o thispermit. The permittee is not relieved of this requirement if he abandons the project without having ,the permit -:transferred to a tnird party. NOTE: No dredging was requested and none is -authorized by this permit. The permittee is ­ advised that since t!J s is a primary nursery area, future :dredging is prohibited. _- NOTE: The permittee should contact the local health department regarding their requirements for septic tank installations (as required under 10 NCAC 10A.1900 et. seq.).
