Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowners).
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistent with the North Carolina Coastal Management Program.

We hereby propose to furnish materials and labor necessary for the satisfactory completion of: BULKHEAD / DOCK
Installation of approximately 124 ft. of wooden Bulkhead wall refaced in front of existing wall. All bulkhead material will be
2.5 marine grade. Bulkhead wall will be approximately 5 ft. out of ground and 5 ft. in ground with fabric installed on landward
side, 6x6x12 posts. spaced 5 ft. 4 in . on center, with 5/8" galvanized tie back rods and anchor blocks for proposed wall support.
Installation of a •1600 boathouse roof with (2) 3 ft. wide by 30 ft. long finger piers on both sides of proposed roof structure.
Installation of (4) additional pilings for secondary lift. All material for boathouse roof will be treated material with 3 dimensional
shingles to coincide with house. All headers on proposed finger piers will be thru-bolted with 5/8" galvanized bolts and decking
will be installed with 3 1/4" stainless ringshank nails. All piling will be installed to a minimum of 10 ft. into canal bottom.
Installation of owners (2) Tide Tamer boatlifts. All State, County, and City of New Bern permits, are included in this contract.
All material is guaranteed to be as specified, and the above work will be performed in a workmanlike manner in accordance with the
rawings and specifications submitted. All wort- will conform to all state, county, and local codes, permits, and guidelines. This

ACCEPTANCE OF PROPOSAL
The above prices, specifications, and conditions are satisfactory and are hereby accepted. You are authorized to do the work as specified. Payments will be made
as outlined above. Before the job begins, the Owner shall furnish the Contractor with the location and character of any underground utilities not located by the
locating service. If Owner shall fail to do so, Owner agrees to release, indemnity, save harmless and defend the Contractor from and against all liability, loss,
damage, and expense caused or occasioned thereby. If Owne to complete the payment schedule as outlined and an attorney is hired for collection, Owner
