Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

Notes:
1) All lumber to be treated marine grade southern pine or better.
2) Piles to be 12" diameter butt end or 12" square and driven to 10 ton capacity or refusal.
3) Girders to be 12X12. Attach girders to piles with (2) PS418 Plates with (4) 3/4" thru bolts.
4) Joists to be 4X10" at 16" o/c except as noted on plan. Provide 2X10 blocking between joists at girders. Attach joists to girders with MTS12
straps. Double joists to be nailed together with two rows of 40d nails at 12" o/c and staggered.
5) Decking to be 2X8. Provide (4) 2X8s over and perpindicular to decking for wheel lanes.
6) All straps, hardware, bolts, nails, etc. to be HDG.
7) Height of bridge to be field determined but not to exceed 6' above existing grade.
8) Provide 2X10 cross bracing between piles attached with (4) 3/4" thru bolts.
9) Maximum load limit - 10 tons.
