violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .

over Middleton Creek, Edenton, North Carolina.

copies are not returned to this office before the initiation of development, you will be working without
authorization and will be subject to a Notice of Violation and subsequent civil penalties.

The NCDOT request the issuance of a General Permit .2300 for the replacement
of Low Impact Project - Bridge # 01 in Chowan County. The bridge is located on SR
1100 approximately 1.07 miles east of the intersection of SR 1102. The bridge spans
Middleton Creek, which is a tributary to the Yeopim River. Middleton Creek is classified
by DWQ as class "C, Sw" waters. Bridge # 01 is currently a wooden structure with a
concrete deck that is 34 ft. long by 25 ft. wide. This structure will be removed using
NCDOT's guidelines for bridge removal. The proposed bridge will be replaced on its
existing alignment. The new bridge structure will be 55' in length by 25' wide and built
with precast cored slab concrete deck, concrete piles and concrete end bents. An off -site
detour will be utilized during construction. There are no coastal wetlands associated with
this project.
This bridge is following the Low Impact Bridge Process established by FHWA,
NCDOT and Regulatory Agencies. Included with this letter are the location map, permit
drawings, MP-1 form and Low Impact Data Sheet. If you have questions please call me at

5. Activities and Impacts

This project will provide safe transportation along SR 1100 by replacing an existing bridge crossing over Middleton Creek.
Once the bridge is replaced the roadway facility will continue to provide connectivity and safe access for the traveling public
living and traveling through this area of Chowan County.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
The bridge will be replaced using top down construction techniques utilizing precast concrete structures (piles,caps,deck).
The new bridge will be replaced on its existing alignment, with additional width to allow for current design standards and
safety improvements. The bridge will be constructed using a crane, excavator, dumptrucks, pavers and various other types
of construction equipment.
d. List all development activities you propose.
The existing 34' long x 25' wide bridge will be entirely removed using NCDOT guidelines for bridge removal. The new bridge
will increase the horizontal floodway by extention to a 55' long x 25' wide bridge. The new bridge will consist of 1 concrete
span supported by concrete end bents and girders. This bridge will be replaced on the existing alignment, causing a
temporary road closure. Traffic will be detoured along available routes. Wetland Impacts have been avoided and minimized
to the maximum extent practicable. The resulting impacts are 5.1 sq. feet of permanent suface water impacts and 253 sq.
feet of temporary wetland impacts associated with hand clearing for erosion control device maintenance area. The removal
of bridge approach fill will restore flood lain elevations as shown in the bride profile drawing.
