This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available

additional pages. The purpose of this project is to
maintain the bridge crossing over the Pungo
River Canal. The existing bridge is deteriorating
and needs to be replaced. This work will be
accomplished by removing the old bridge with a
crane in sections. The new concrete slab will be
placed on piles. These piles will be driven with a
vibratory hammer.

In addition to the completed application form, the
following items must be submitted:
• A copy of the deed (with state application only) or
other instrument under which the applicant claims title
to the affected properties. If the applicant is not
claiming to be the owner of said property, then
forward a copy of the deed or other instrument under
which the owner claims title, plus written permission
from the owner to carry out the project.
• An accurate, dated work plat (including plan view
and cross -sectional drawings) drawn to scale in black
ink on an 8 1/2" by 11" white paper. (Refer to
Coastal Resources Commission Rule 7J.0203 for a
detailed description.)
Please note that original drawings are preferred and
only high quality copies will be accepted. Blue -line
prints or other larger plats are acceptable only if an
adequate number of quality copies are provided by
applicant. (Contact the U.S. Army Corps of
Engineers regarding that agency's use of larger
drawings.) A site or location map is a part of plat
requirements and it must be sufficiently detailed to
guide agency personnel unfamiliar with the area to
the site. Include highway or secondary road (SR)
numbers, landmarks, and the like.

ADJACENT RIPARIAN LANDOWNER STATEMENT
( Hyde County: Bridge replacement on NC 45 over Pungo Canal)
General Statute 113-229 requires that riparian landowners with property adjoining a
proposed development in an Area of Environmental Concern (AEC) be given thirty (30)
days in which to comment on the proposed development. This form allows the adjacent
riparian landowner to express either: (1) that he objects to the project; or (2) that he does
not object and desires to waive his/her right to the 30-day period so that the processing of
the application can progress more rapidly. Of course, the adjacent riparian landowner
need not sign this form at all if he/she so chooses. No response within 30 days
following receipt of this letter and form will be construed to mean the adjacent
riparian landowner has no objections.
I, EU,S ER Co • am an adjacent riparian property owner and am
aware of the North Carolina Department of Transportation's plans for the replacement of
a bridge on NC 45 in Hyde County over the Pungo Canal. I am further aware that this
work will occur in one or more AEC's and therefore will require authorization from the
Division of Coastal Management in accordance with the Coastal Area Management Act
(CAMA).
I have no objection to the project as presently proposed and hereby waive
that right of objection as provided in General Statute 113-229.
I have objections to the project as presently proposed and my comments
are attached.
