This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythat this project is consistentwith the North Carolina Coastal Management Program.

Subject: Nationwide 12 Permit Application and CAMA General Permit Application
for utility work due to the proposed replacement of Bridge No. 24 over New
River on US 17, in Onslow County. Federal Aid Project No. BRSTP-
0017(34), TIP No. B-4214. Debit $200.00 from WBS 33560.1.1.
Please find enclosed the permit drawings, certified mail receipts, and MP forms for CAMA
General Application for the above referenced utility work. The NCDOT proposes to replace
existing Bridge No. 24 on US 17 over New River in Onslow County. Associated utility work
needs to be completed prior to the start of bridge construction. The Telecome
communications cable attached to Bridge 24, which is being replaced, will be directionally
bored at least 6-feet under the New River thalweg to maintain operation. Existing overhead
electrical powerlines located on the north and south side of Bridge 24 will be replaced with
overhead powerlines located on the south side of Bridge 24. 50 poles will be removed from
the project area and 16 new poles will be installed.
Due to the project's schedule and need to expedite utility relocations, NCDOT is proposing
to separate the utility work necessary for the project from the majority of the actual bridge
replacement. To accomplish this, permitting is being requested via this Nationwide Permit
12 and LAMA General Permit application for utility work. A Nationwide Permit 23 and
CAMA Major Development Permit application will be submitted under separate cover for all
remaining work associated with the bridge replacement.
There will be two square feet of temporary fill to wetlands adjacent to New River and 10
square feet of existing channel impacts to the New River from pole removal. Traffic will be
detoured off -site along surrounding roads, during construction.

The Telecome communications cable attached to Bridge 24, which is being replaced, will be directionally bored at least 6-
feet under the New River thalweg to maintain operation. Existing overhead electrical powerlines located on the north and
south side of Bridge 24 will be replaced with overhead powerlines located on the south side of Bridge 24.

Typical directional methods and techniques will be utallized to do a single bore. Powerpole removal and instillation will
utalize typical construction methodes to be determined by the contractor.

Single directional bore of Telecome communication cable. Removal of 50 powerline poles and assocated wires to be
replaced with overhead electrical lines located on the southside of Bridge 24 crossing the New River. Two powrlines will
