Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .

This letter is requesting a modification to general permit .2300 (# A53456) be issued for the addition
of rip -rap shoreline stabilization at each corner of the culvert. The original permit drawing did not depict
any shoreline stabilization beyond the culvert head walls. It became apparent during the construction
process that high water erosive forces and destabilization of the creek banks would cause future erosion
problems adjacent to the culvert headwalls. Therefore, NCDOT is proposing to place additional rip -rap as
depicted on the attached permit drawings to prevent bank scour and maintenance problems. This work will
follow standard protocol for the placement of rip -rap. In addition, the southeast corner will need some bank
scour repair, which occurred during a storm event. The creek bank scoured approximately a 20 ft. long
section adjacent to the outlet end headwall on the southeast corner. In order to properly fix the bank and
continue to allow ground water flow through the bank, NCDOT is proposing 2-3 ft. of granite stone, a layer
of filter fabric, 4-5 ft. of earthen material and plate with Class I rip -rap.
Included with this request is a location map, copy of existing permit, and modified permit
