This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become

need your guidance on as well. The DOT is proposing to replace a culvert on Adams Creek Road where a canal to Kellum
Creek crosses under the road. Kellum Creek is designated as a PNA. I will require that they use turbidity curtains on the
project. What is your guidance regarding any kind of moratorium for the project.

Hey Mr. Stephen .... I called Brad Price today .... Brad said that the existing pipe had no cover
on it; so they decided on a smaller pipe and the proposed pipe could handle the water for the
mosquito ditches that had been dug on the West side of the road. I'm working on the permit
application for the pipe replacement on SR 1002,Bay city Road,Beaufort County. I have written
that the water line is the CAMA Line at this site; is this correct? ... I've attached a map for

Hi Jay and Mike,
In reviewing the application for the CAMA general permit for the culvert replacement on SR1700 (Adams Creek Road) in
Craven County, I noticed that the proposed culvert is smaller than the existing culvert. I have never seen DOT propose
to replace an existing culvert with one that was smaller. Just wanted to check and make sure that this was not a
miscalculation or typo. Is the proposed 53" x 41" appropriately sized for the estimated hydraulic flow? Also the stream
is designated as a Primary Nursery Area by the DMF and will likely be subject to a moratorium period (probably ending
on June 15), are there any emergency replacement concerns associated with this culvert?

Erosion Control Schedule
• Install erosion/sediment control measures according to plans.
• Grading of slopes; '1.5: 1'= stabilize with Class B riprap
• Seed all disturbed areas as soon as final grading is completed.
• Clean out and/or rework all temporary sediment control measures after
significant rainfall event (or as otherwise needed). These measures 'should be
maintained until a'permanent vegetative cover is established and project is
complete
Pipe Installation Schedule
• Install erosion/sediment control devices per plans.
• Remove material while limiting, as much as possible, material and sediment
from entering the stream and/or escaping from the project.
• Prepare pipe foundation while taking care to limit material and sediment from
entering the stream and/or escaping from the project. Where possible new
grade shouldbe below the existing streambed. Bedding material will be washed
stone.
• Place new pipe and compact fill.
• Install slope protection on outlet and inlet ends of pipe (Class B Granite riprap)
• Establish permanent vegetation as soon as possible
• Remove sediment control devices after project has stabilized and permanent
vegetative cover is established.
