drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro -
is consistent with the local land use plan and all local
ances, and 2) a written statement has_been obtained from

Dear Mr. Mercer:
Thank -you for arranging to come out to my home today. I've
enclosed a completed General Permit Application with a check for
$50.00, as well as the responses from my neighbors. I've also
included a copy of the return receipt from Mr. Respass.
Once the pier has been constructed I intend to build a wooden
walkway from the pier to the retaining wall. The walk would be
four feet wide and not more than three feet above the ground. Do
I require a permit from your department fore this walkway? If
so, please consider this a request for that permit. I am aware I
am required to notify my homeowner's association of my intent to
construct this walkway, and I intend to comply with that
requirement.
I trust you now have all the information you will need to process
