This permit is subject to compliance with this application, site
drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro-
ject is consistent with the local land use plan and all local
ordinances, and 2) a written statement has been obtained from
adjacent riparian landowners certifying that they have no
objections to the proposed work.

Notes,
Tho "structures" located within 75 feet of MHW will be the existing monitoring wells
MW 4, MW 6, and MW 12; a planned recovery well near MW 4; and the LPP distribution
systems. The J-PP systems are designed to create a water mound between the majority of
the free product plumes and the surface waters. All the structures are temporary and will
ba removed when remediation Is complete. Initial operation will involve recovery of the
free product near MW 4 prior to placing the LPP system Into operation. It is likely that : 4
the recovery well between the LPP system at MW 4 and the stream bank will not be used or oe
the froe product recovery is completed at that location. -4 4,;
