Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certify that this project is consistentwith the North Carolina Coastal Management Program.

Ref: Three (3) Hurricane Damage Houses belonging to Carteret General Hospital
(Taylor Extended Care)
Scope of Work:
Demo and remove from site homes @
1. 28' x 54' brick house with asphalt shingle roof @ 506 Hwy 70 — Sea Level.
2. 28' x 60' brick house with asphalt shingle roof @ 113 Bayshore Drive —Sea Level
3. 30' x 60' cement block with asphalt shingle roof @ 119 Bayshore Drive —Sea Level
All homes to include removal of footings, concrete driveways, and carport concrete slabs.
Contractor to deliver and spread one (1) to two (2) loads of clean dirt to fill in holes that
may be left after removing footings. Dirt is also used to keep water from ponding in the
old house footprint. Surrounding area of homes will not be touched. Trees, grass, shrubs,
ect.
